Request for
Return Materials Authorization

All Factory repairs must have a Return Materials Authorization
number (RMA).

The repair prices include fault isolation, replacement of faulty components and test to factory specifications. Factory Repair will carry a warranty of 180 days in accordance with the current terms and conditions.

All equipment returned must be intact and include all parts. We will not attempt to repair equipment which has been returned in pieces or is missing significant parts. Include a description of the failure or malfunction with the returned equipment in order to ensure that the problem will be fully addressed.

To Process your repair:

  • You must have a Sensidyne issued RMA before you send in your equipment
  • Return completed RMA Request Form to Service@Sensidyne.com
  • We must have your Tax status on file
  • Please include a copy of this form when shipping your instrument
  • Terms are Net 30 for Domestic customers and Net 60 for International – Subject to approval
  • There will be a 3.5% processing fee charged for all credit card transactions.
